host descriptionPatient presents with hyperkalemia with echocardiogram changes and acute exacerbation of untreated chronic kidney disease (admission serum creatinine > 17 mg/dL). Clinical course was complicated by acute anemia (hemoglobin 5.7 on admission), pneumonia with extensive cavitary lesions secondary to either aspiration or lung abscess and staphylococcal bacteremia secondary to either a recently-drained sacral abscess or pneumonia. There remains a high clinical suspicion for infective endocarditis despite an unremarkable transesophageal echocardiogram. Patient was stabilized, managed medically with ampicillin-sulbactam and discharged home on amoxicillin-clavulanate with outpatient followup. Duration of bacteremia: 8 days.
